Submit your application below and, at your earliest convenience, place your payment for $10 in cash or check in an envelope marked with you name into the PAC mailbox outside the library in Independence Center. A monitor can help you find the mailbox. Checks must be made payable to “Sun City Anthem Community Association, Inc.” exactly as shown.
